I am required by law to follow the Code of Ethics of Licensed Professional Counselors. A copy of the Code of Ethics is available to you upon request. Material revealed in counseling will remain confidential. Exceptions to confidentiality, in accordance with state law, include:
1. Information contained in a signed Authorization to Release Information, indicating informed consent of such release,
2. Your expressed intent to harm yourself or someone else,
3. There is reasonable suspicion of abuse and/or neglect against a minor child, elderly person (65 or older), or a dependent adult.
4. A court order is received directing the disclosure of information.
